After booking and prior to your initial consultation, we require that you complete and sign your patient registration forms.

To make this process more convenient, we will send you an email with instructions and a unique access code, along with a link to complete your forms online. This link is also available below. After you have completed the form, please ensure to press the Submit button at the bottom to automatically send us your information. 

Filling out your forms at home allows us time to review your medical history before your visit and helps ensure a smooth appointment.

If you prefer, you may arrive early to your appointment to complete the forms in our office. Please inform our staff in advance if you choose this option, and being a list of all medications to your appointment. 

The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
